The Bergendahl Institute was established to transfer to the Healthcare Industry, 25 years of proven human error management techniques from the U.S. Nuclear Power Industry. The Institute's mission is to reduce medical errors, improve patient safety programs and strengthen safety cultures.

The techniques used were developed through the sharing of experiences, best practices and lessons learned with other industries such as the Aviation and the Petrochemical Industries.

The Institute team is made up of nuclear power and healthcare executives, managers and performance improvement professionals dedicated to rapid improvement in patient safety without the trial and error period that was required to attain advances in other industries.


According to the Institute of Medicine;

  • More people die each year from medical errors than from motor vehicle accidents, breast cancer or AIDS
  • From 1983 to 1993 there was a 250% increase in the number of deaths caused by medication errors
  • As of 2003, JCAHO reports the number of Sentinel Events (unexpected deaths and serious injuries) reported each year continues to rise

Conversely, in the Nuclear Power Industry, since the Three Mile Island accident in 1979, the World Association of Nuclear Operators reports;

  • The number of significant events continue to decrease and was 45 times lower in 2000 than in 1989
  • The Industrial Safety Accident Rate (workplace injuries) is nearly 10 times lower than it was immediately following the Three Mile Island accident due to error reduction practices
  • In fact, today the U.S. Nuclear Power Industry has a lower accident rate than the Finance, Insurance and Real Estate Industries combined

At the same time safety has improved in the Nuclear Industry, production costs have gone down.
